Simply put Islam is the religion.
A person that practices the religion is a Muslim.

Or as it is stated on several Islamic sites:

Islam means submitting to Allaah, worshipping Him alone and obeying Him, and shunning all other religions/faiths as well as those who believe in the other faiths.

The person who enters this religion is called a Muslim (one who submits), because he has submittedto all the rulings that have come from Allaah and from the Messenger of Allaah.
